In a skilled nursing facility (SNF), delivering exceptional care takes more than individual expertise—it requires seamless collaboration across disciplines. One of the most effective ways to foster that collaboration is through well-structured rounds supported by clear, consistent communication plans. Why Communication Plans Matter in Rounds

Environmental Rounds

Purpose

Environmental rounds assess the physical safety, cleanliness, and functionality of the facility. These inspections are crucial for regulatory compliance, infection control, and resident comfort.

Scope and Frequency

  • Conduct environmental rounds monthly at minimum, with targeted follow-ups as needed.
  • Cover resident rooms, bathrooms, hallways, dining areas, and outdoor spaces.

Roles and Responsibilities

  • Lead: Maintenance or environmental services manager
  • Support: Nursing, housekeeping, infection preventionist
  • Regulatory oversight: Involve the administrator or safety officer for compliance review.

Documentation Tools

  • Standardized forms noting hazards, cleanliness concerns, temperature / humidity readings, and lighting.
  • Photos for reference when reporting maintenance needs.

Information Sharing

  • Provide a same-day report to maintenance and housekeeping teams.
  • High-priority safety concerns (e.g., trip hazards, malfunctioning fire doors) should be communicated immediately via phone or in-person alert.

Follow-Up and Feedback

  • Track each work order until completion, with confirmation to the reporting staff.
  • Use quarterly safety committee meetings to review recurring issues and prevention strategies.

During rounds, a team notices a hallway light is out near a high-traffic area. It’s reported immediately, logged in the maintenance system, and repaired within the hour—preventing a possible fall risk.

Nursing Rounds

Purpose

Nursing rounds focus on clinical care, ensuring each resident’s care plan is being followed and updated as needed. These rounds may be led by charge nurses, unit managers, or nurse practitioners.

Communication Plan Elements

Scope and Frequency

  • Daily nursing rounds for high-acuity residents.
  • Weekly rounds for stable residents to review care plans and monitor changes.

Roles and Responsibilities

  • Lead: Charge nurse or nurse manager
  • Support: CNAs, therapists, physicians, dietitians as applicable
  • Resident/family input: Encourage family participation during care conferences.

  • EHR-integrated checklists for vital signs, pain levels, wound assessments, medication adherence, and care plan updates.
  • Quick-reference notes for follow-up during shift changes.

Information Sharing

  • Handoff reports at shift change include any findings from nursing rounds.
  • Urgent concerns (e.g., sudden weight change, pressure injury risk) are communicated directly to the provider.

A nurse observes redness on a resident’s heel during rounds. The CNA is instructed to use a heel protector immediately, the wound nurse is notified, and the care plan is updated to include daily skin checks.

Integrating the Three Types of Rounds

While each type of round has a unique focus, integration is key to true interdisciplinary care. Here’s how:

  4. Resident and Family Involvement – Include residents and families in the loop on any actions taken as a result of rounds to build trust and engagement.

A good communication plan doesn’t stop at information sharing—it measures results. Track:

  • Completion rates for action items identified in rounds.
  • Reduction in repeat issues (e.g., recurring maintenance problems).
  • Resident satisfaction scores related to safety, comfort, and quality of care.
  • Staff engagement in interdisciplinary meetings.
